Right now, we are looking for an experienced Registered Nurse to become part of the team here in Timaru. In this role, you will work in a dynamic and equitable health environment, which promotes the development of advanced interdisciplinary healthcare based upon service specifications and contracts of each respective professional discipline. All to meet the needs of the community.
You, as the Integrated Community Services Liaison, will work collaboratively with Primary and Secondary Services to coordinate care for clients referred into Community Services via the ICATT single point of entry. Additionally, you will contribute to planning by highlighting challenges and considerations for complex admission and discharge to the Services, with minimal disruption to the client, which covers health as well as social / support needs.
Consolidated nursing practice is a required with excellent communication skills alongside, high level of IT competency are vital specifications for this role. Experience working in the community is also a vital requirement. This an excellent opportunity to broaden your nursing experience as we enter a new direction for Health care in New Zealand.
Applicants are required to be registered with the New Zealand Nursing Council and have a current Annual Practising Certificate and full driver’s license.
