About the Role

As part of a diabetes CNS team, this role works collaboratively with other multi-condition hub members and other health professionals to provide coordinated, tailored health promotion to patients, family, whanau and caregivers.  The role also provides specialized education and mentoring of other health service professionals.

About You

You will be involved with initiating and or participating in evidence-based service improvements, quality enhancement, policy development and building of staff capability.

Clinical aspects of this role will include face to face clinics with patients with diabetes across the age continuum.

Key Requirements Include:

  • Nursing Council of NZ registration, and current APC
  • Have be working towards a post graduate diploma 
  • Significant relevant clinical experience
  • Be able to work collaboratively with health professionals and work as an autonomous practitioner. 
  • Excellent clinical assessment skills
  • Current New Zealand Driver's License

Salary Range

This role is covered by the terms and conditions of the relevant Nursing Collective, Grade 2 Senior Nurse Scale, step determined upon relevant experience.
